A case of a diabetic woman with an uncontrollable arm

We present the case of an 83-year-old female patient with left-side hemichorea and hemiballism associated with poorly controlled type 2 diabetes mellitus. Hyperintensity in the right basal ganglia was evident in T1-weighted brain magnetic resonance. Clinical and radiological findings were consistent with diabetic striatopathy.

Nitrogen-Phosphorus-Potassium containing liquid fertilizer intoxication presenting with extreme hyperkalemia, metabolic acidosis and ECG changes

Liquid fertilizers are widely used for fertilizing in- and outdoor vegetation. Despite the easy accessibility and widespread use, serious intoxications are rare.
This case report describes a 61-year-old woman who was treated for life-threatening hyperkalemia, metabolic acidosis and ECG changes after intentional ingestion of liquid fertilizer. Our case shows that intake of liquid fertilizer, though infrequent, can cause serious, life threatening complications.

Acute coronary syndrome after initiation of amoxicillin/clavulanate

We report the case of a 54 year old man referred to the Emergency Department for rapid onset of an itchy rash and oppressive epigastric pain after assumption of amoxicillin/clavulanate. Electrocardiogram aand laboratory findings were consistent with acute coronaty syndrome. After coronary angiography, diagnosis of type II Kounis syndrome was made.

Hypomagnesaemia Induced Reversible Cerebellar Syndrome

Hypomagnesaemia may cause neurological symptoms as part of its presentation. This case demonstrates one such unusual example of a reversible cerebellar syndrome resulting from magnesium deficiency. An 81-year-old woman presented to the emergency department with a history of chronic tremor and other cerebellar signs. Severe hypomagnesaemia was detected within her initial biochemistry results. Correction of this deficiency led to a resolution in her symptoms.

Mind the bubbles – A patient presents with seizure after haemodialysis

A patient with patent ductus arteriosus presents with seizure after haemodialysis. Although conscious on arrival to the emergency department with spontaneous limb movement, he develops recurrent convulsion and left hemiparesis after admission. The approach to the haemodialysis patient presenting with seizure is discussed and the role of early hyperbaric oxygen therapy for an uncommon but important diagnosis is highlighted.

A man who became breathless after tiling a bathroom

Inhalational lung injury should be considered in patients presenting with acute respiratory symptoms and a history of occupational or recreational exposure to toxic substances. We present the case of a 29-year-old patient who developed pneumonitis following usage of waterproofing sealant spray in an enclosed space, despite the use of a dust mask. The patient was managed with oxygen therapy, corticosteroids and bronchodilators. He made a complete clinical recovery with resolution of almost all changes seen on computerised tomography (CT) imaging within 7 days.

Sepsis secondary to a misplaced urinary catheter into the ureter

Misplacement of a urinary catheter in the ureter is a rare phenomenon. The described cases occurred in patients with neurogenic bladder. We describe an unusual case of 58 years old female where the Foley catheter was unintentionally placed in the left ureter. The patient developed sepsis due to complete obstruction of the left ureter treated with antibiotic therapy after repositioning the catheter.
