Abstract

Chest pain is an extremely common presenting complaint on the acute medical unit. It is important to distinguish between patients who have serious pathology and those without. Often, the focus is on ruling out an acute coronary syndrome and inadequate consideration is given to other possible causes.

This case highlights the importance of performing relevant investigations in a timely manner, in order to ensure that a correct diagnosis is made.

Aortic dissection: a review of the diagnosis and initial management

Abstract

Aortic dissection is a medical emergency which carries a high mortality, particularly if undiagnosed and untreated. The diagnosis may be overlooked because the ‘classical’ history, examination and imaging signs are present in only a minority of cases. After reflecting on the underlying pathophysiology, this review examines aspects of the history and examination which should alert Acute Physicians to a possible diagnosis of aortic dissection. The relative benefits of different imaging techniques are summarised, along with the prognosis and treatment for different types of aortic dissection. Finally a case of aortic dissection which went undiagnosed for a month is presented to illustrate some of the key teaching points raised in this article.

Abstract

Thoracic aortic dissection may present in many different ways leading to delayed diagnosis, often with catastrophic results. We present a patient with sudden onset epigastric pain, breathlessness, fever and a left sided pleural effusion, who was initially treated for a chest infection, subsequently for pulmonary embolism, with a serendipitous diagnosis of aortic dissection eventually being made. The strength of association of pleural effusion and aortic dissection and the need for vigilance with regard to the diagnosis of aortic dissection is emphasised.
